Onyx & Your Medical Practice
Our team of mental health experts streamlines the entire behavioral health care process for your patients. Once the primary care physician determines a patient qualifies for additional care, our behavioral health care managers (BHCM) meet with patients (via telehealth) after their PCP visit to assess and provide brief interventions. Our BHCM will then send detailed assessments to our psychiatric consultant, a licensed doctor of clinical psychology, using our secure proprietary software (MedMerge360). Our psychiatric consultant will then review the assessment and make suggestions as needed i.e. changes to medication. Finally, all of the updated patient information is seamlessly uploaded into the PCP’s EMR. Patient-centered, cost-effective, innovative: that’s the Onyx Model.

The platform enables our care team to document comprehensive patient health updates, including goals, barriers, and interventions, all within a structured care plan.
